This Terms and Service Agreement (“Agreement”) outlines the responsibilities, rights, and expectations associated with the renewal of certification under Geek Therapeutics’ credentialing program. By submitting the renewal form and associated materials, you agree to the terms outlined below.Eligibility for Renewal To be eligible for annual renewal, all Certified Geek Therapists and Specialists must: Hold an active and unrestricted professional license relevant to their scope of practice. Complete at least 12 CEU hours through Geek Therapeutics-approved programming within the past 12 months. Submit proof of 1 CEU in Ethics from any accredited continuing education provider. Provide an updated professional headshot and verify contact information. Credential Validity & Expiration Credential renewal is valid for 12 months from the date of approval. Failure to renew by December 31st of the current calendar year will result in the expiration of your certification. Lapsed credentials require a reinstatement application and associated fees to regain certification status. Accuracy of Information You certify that all information submitted in your renewal form is accurate and truthful. Any falsification or omission may result in: Denial or revocation of credentials Reporting to relevant professional boards if required Ineligibility for future certification Use of Certification & Logo Certified individuals may use the Certified Geek Therapist or Specialist title and associated logo only while credentials are active and in good standing. Any misuse of titles or marks after expiration may result in legal or professional action. Confidentiality and Data Handling Geek Therapeutics will store your renewal documents and personal data in accordance with industry best practices and HIPAA-adjacent confidentiality standards. Your information will not be shared or sold. Documents are retained for 6 years per CE provider requirements. Non-Transferability Credentials are non-transferable. Certification is granted to individuals only and may not be used by institutions or third parties as proof of endorsement. Right to Modify Terms Geek Therapeutics reserves the right to revise this agreement at any time. Notification of changes will be sent to the email address on file. Continued use of your certification after changes implies acceptance of revised terms. Acknowledgment By signing below or submitting your renewal form electronically, you acknowledge and agree to all terms listed in this Agreement.
